Boutique Product Sales Registration Form for 2024 Quilt Show

 Boutique Product Registration Form – available August 1 – September 13, 2024

You must be a member in good standing by September 13, 2024 to register items for the Boutique. Members that wish to sell products in the boutique should be willing to volunteer to work at least one shift during the quilt show.

Please submit one form for each unique item, or each category of items (where you have multiples) that share the same price.

The name of your sales item is required, i.e. needle case, pin cushion, placemats, etc.  Also, the approximate size in inches will help us identify the needed space for your items.

The units available means all units of the same/similar item that are also the same price.  If you have placemats for sale (4 in a set) and you have 3 sets all at the same price then your units available will be 3.  If you have a similar item but a different price then you must submit that item on a new entry form.

For any donations we also require that you use the entry form. Please include what you feel the price should be.
